Requests for certified copies of a vital record may be processed either in person at the address listed below, or by mail, if the appropriate information, requirements, and payment are mailed to:

The application for certified copies of a vital record are posted in the forms box on the left of this page or can be obtained in person at our office. We cannot process a request without this completed form. Applications for vital records can be accepted between the hours of 9 a.m. and 3 p.m.
Fees: $20.00 for 1st copy, and $5.00 for each additional copy of the same record at the time of application.
Acceptable identification is required. A valid and current photo driver’s license or a valid and current county ID are acceptable. Please note that “valid and current” mean with the address where you reside at the time of your application and note expired. If you do not have either, you will need two alternate forms of identification such as vehicle registration card, passport, utility bill, bank statement (current within 90 days).More information can be obtained from the NJ Department of Health, Vital Statistics page.
